Modern Biological Interpretation of Traditional Chinese Medicine Syndromes:Insights into Inflammation–Cancer Transformation in Gastric Precancerous Lesions from the Perspective of Spleen–Stomach Dampness-Heat Syndrome

    Abstract: Precancerous lesions of gastric cancer (PLGC) represent a critical stage in the development and progression of gastric cancer. Their progression is closely associated with persistent chronic inflammation, gastric microbiota dysbiosis, metabolic reprogramming, and epigenetic abnormalities. Spleen–stomach dampness-heat syndrome is one of the common traditional Chinese medicine syndrome patterns observed in patients with PLGC, and is frequently associated with Helicobacter pylori infection, gastric mucosal inflammatory activity, intestinal metaplasia, and other pathological changes. Modern studies suggest that spleen–stomach dampness-heat syndrome may correspond to a pathological state characterized by a pro-inflammatory microenvironment, increased metabolic burden, and microbiota dysbiosis. It may participate in gastric mucosal inflammation–cancer transformation through immune-inflammatory imbalance, metabolic abnormalities, and molecular regulatory alterations. This article reviews the mechanisms underlying the association between spleen–stomach dampness-heat syndrome and inflammation–cancer transformation in PLGC from the perspectives of gastric microbiota, immune inflammation, metabolic reprogramming, and epigenetic regulation, aiming to provide a reference for syndrome-based stratification and interventions based on clearing heat and resolving dampness in PLGC.
