Abstract: Gastric precancerous lesions are critical pathological stages in the progression from normal gastric mucosa to gastric cancer, including atrophy, intestinal metaplasia, and dysplasia. Preventing and treating gastric precancerous lesions can effectively reduce the incidence of gastric cancer. Modern pharmacological research indicates that the main active components of the traditional Chinese medicine Panax notoginseng, such as saponins and flavonoids, can inhibit the progression of gastric precancerous lesions and treat gastric cancer through multiple pathways. These mechanisms include inhibiting abnormal cell proliferation, inducing apoptosis, regulating autophagy, alleviating inflammatory responses, repairing the gastric mucosal barrier, combating oxidative stress, and inhibiting angiogenesis, demonstrating characteristics of multi-target, multi-pathway, and multi-level actions. Current research on the mechanisms of Panax notoginseng against gastric cancer and gastric precancerous lesions is mostly focused at the cellular level, with significant deficiencies in animal-related experiments and specific clinical studies. Furthermore, besides notoginseng saponins, there is limited research on the mechanisms of other active substances such as polysaccharides and flavonoids against gastric cancer and gastric precancerous lesions. Meanwhile, the interactive regulatory network among various signaling pathways has not yet been elucidated, and systematic research on component-target-pathway interactions urgently needs to be conducted. This article reviews the active components and mechanisms of Panax notoginseng against gastric cancer and gastric precancerous lesions in recent years, aiming to provide relevant theoretical foundations for deepening research in this field.
