Abstract: Precancerous lesions of gastric cancer represent a critical pathological stage in the progression from chronic gastritis to adenocarcinoma, encompassing intestinal metaplasia (IM) and dysplasia of various grades. Although eradication of Helicobacter pylori can reduce the risk of gastric cancer, it lacks effective intervention for the molecular events that have already occurred. Traditional Chinese medicine monomers, with their well-defined structures, controllable quality, and multi-target actions, have made significant progress in the prevention and treatment of PLGC in the past five years. This article systematically reviewed the Chinese and English literature from PubMed, Web of Science, CNKI, and Wanfang databases from January 1, 2020, to May 31, 2025.Focusing on the three core mechanisms of “inflammation-proliferation-signal transduction pathways,” we evaluated the in vitro and in vivo evidence of representative monomers such as ginsenoside Rb1, astragaloside IV, and tetrandrine through evidence-based assessment, aiming to provide new insights for the precise prevention and treatment of PLGC.
