Abstract: Sepsis is a dysregulated host response to infection, characterized by high morbidity and mortality, for which no specific therapeutic agents are currently available. Traditional Chinese Medicine (TCM), with its inherent properties of multi-component, multi-pathway, and multi-target actions, exhibits distinct advantages in the treatment of sepsis. This review summarizes recent advances in research on TCM active components that modulate sepsis through the regulation of inflammatory response pathways, with a focus on the mechanisms of action of bioactive constituents such as flavonoids, alkaloids, and terpenoids. Such mechanisms include inhibiting signaling pathways e.g., nuclear fator-kappa B(NF-κB), toll-like receptors 4(TLR4)/myeloid differentiation primary response gene 88(MyD88), and phosphatidylinositol 3-kinase(PI3K)/AKT, mitigating cytokine storms, alleviating oxidative stress, and improving microcirculation. Network pharmacology and molecular docking technology offer novel perspectives on elucidating the multi-target mechanisms of TCM. Despite the methodological limitations of existing studies, TCM active components serve as a valuable resource for the development of novel anti-sepsis agents.
