Abstract: Objective To investigate the clinical efficacy of Xianglian Pills combined with quadruple therapy in treating chronic atrophic gastritis (CAG) with spleen-stomach damp-heat pattern complicated by Helicobacter pylori (Hp) infection. Methods Eighty patients with CAG complicated by Hp infection and presenting spleen-stomach damp-heat syndrome were randomly assigned to a control group and an observation group, each comprising 40 patients. The Western medicine group received quadruple therapy combined with folic acid tablets, while the Chinese medicine group received quadruple therapy combined with Xianglian Pills. Both folic acid tablets and Xianglian Pills were administered for 16 weeks, and quadruple therapy was administered for 2 weeks. Comparisons were made between the two groups regarding changes in gastroscopic pathological scores, TCM symptom scores, pepsinogen I (PGⅠ), pepsinogen II (PGⅡ), and gastrin-releasing peptide 17 (G-17) before and after treatment. Hp eradication rates and adverse reaction occurrences were also recorded. Results Post-treatment gastroscopic pathology scores and TCM symptom scores decreased significantly in both groups compared to pre-treatment levels (P<0.05). PGⅠ and G-17 levels significantly increased (P<0.05), while PGⅡ levels significantly decreased (P<0.05) compared to pre-treatment levels. The observation group demonstrated superior improvement in endoscopic pathology scores and TCM symptom scores compared to the Western medicine group (P<0.05), and superior elevation of PGⅠ and G-17 levels alongside reduction of PGⅡ levels (P<0.05). Comparison of Hp eradication rates between groups after treatment revealed statistically significant differences (P<0.05). Conclusion Xianglianwan demonstrates favorable clinical efficacy in treating spleen-stomach damp-heat pattern CAG with Hp infection. It promotes gastric mucosal pathological repair, alleviates symptoms, improves gastric microenvironment, regulates pepsinogen and gastrin levels, and exhibits high safety.
