Abstract: Helicobacter pylori (Hp) is one of the most common pathogens in the digestive system. Recent studies have revealed that Hp acts as an immunomodulator, participating in the pathogenesis of digestive system autoimmune diseases through multiple mechanisms including molecular mimicry, immune cell regulation, and modulation of gastrointestinal microbiota, with differential effects across various diseases. Eradication of Hp has positive implications for the clinical management of these patients, though individualized decision-making is required based on disease characteristics. This review systematically summarizes the distinct roles of Hp in the development and progression of autoimmune gastritis, inflammatory bowel disease, autoimmune liver disease, and autoimmune pancreatitis, aiming to provide new insights for the diagnosis and treatment of digestive system autoimmune diseases.
