Treating Pulmonary Nodules by Regulating Phlegm and Qi: A Discussion on the Clinical Use of Banxia Houpu Tang

Abstract: This study aims to explore the formula-syndrome correspondence logic in differentiating and treating pulmonary nodules (phlegm-qi entanglement pattern) based on the theory of simultaneous treatment of phlegm and qi, and to elucidate the clinical extension value of Banxia Houpu Decoctionfrom the Jin Gui Yao Luein intervening in pulmonary nodules through regulating qi and resolving phlegm. Grounded in classical Traditional Chinese Medicine theories and historical medical texts, we analyzed the pathological evolution of pulmonary nodules characterized by qi stagnation as the precursor, phlegm coagulation as the core, and blood stasis as the formation. Through a typical clinical case, the intervention effect of modified Banxia Houpu Decoctionon nodule regression was observed. Banxia Houpu Decoction, endowed with pungent dispersion and bitter descent properties, excels at promoting qi circulation, dissipating masses, lowering adverse qi, and resolving phlegm, thereby directly targeting the pathogenesis of phlegm-qi entanglement. Its formulation implicitly aligns with the principle that treating qi equates to treating phlegm, effectively interrupting the pathological progression of qi stagnation – phlegm coagulation – stasis formation via mild qi transformation. Clinical modifications based on syndrome differentiation not only stabilized but also reduced nodule size without significant toxic side effects, demonstrating the clinical wisdom of treating different diseases with the same method and providing robust theoretical and clinical evidence for Traditional Chinese Medicine interventions in pulmonary nodules.
