Modern Reconstruction of the TCM Theory "Physical Trauma Leads to Swelling" from a Cross-Scale Homeostatic Perspective

    Abstract: The traditional Chinese medicine (TCM) theory of "structural injury leading to swelling" elaborates on the internal relationship between structural damage and fluid metabolism disorders, and is an important part of the classic TCM pathogenesis theory. It is widely applied in the clinical diagnosis and treatment of bone injuries, arthralgia, edema, sores, and other diseases [1]. This paper takes the theory of form and qi as the core and systematically defines the TCM connotations of "form" and "swelling" and their corresponding modern biological characterizations from a multi - scale perspective, including the macro - form, tissue barriers, cell structure, and extracellular matrix, based on the research paradigm of cross - scale life homeostasis. The study combs the core pathogenesis chain of "structural injury - qi failing to consolidate - fluid disorder - swelling formation" and clarifies the applicable scope and boundary conditions of this theory, avoiding the problems of concept generalization and circular argumentation. By constructing a theoretical interpretation framework that integrates traditional Chinese and Western medicine, this study reveals the internal law of fluid homeostasis imbalance mediated by structural injury, provides a theoretical basis for clinical syndrome differentiation to distinguish between structural injury swelling and qi failure to transport and transform swelling, and at the same time promotes the modernization, standardization, and testability of the classic TCM pathogenesis theory, providing more accurate guidance for clinical practice [1].
