Abstract: As one of the primary causes of male infertility, oligoasthenospermia is closely related to the dysfunction of “Yang transforming qi, Yin giving form.” While most practitioners focus on treating the kidneys, liver, and spleen—and have achieved certain therapeutic outcomes—the crucial role of the lungs is often overlooked. According to the theory of “Yang transforming qi, Yin giving form,” the lungs govern the qi of the entire body. Through the warming and propelling functions of Yang qi, they direct the process of “Yang transforming qi.” Simultaneously, via their dispersing and descending actions, the lungs coordinate the process of “Yin giving form,” ensuring the smooth formation of Yin essence. The kidneys, as Yin organs, facilitate the condensation, formation, and storage of kidney essence under the nourishment of kidney Yin, resulting in mature semen with specific morphology, quantity, and quality—embodying the function of “Yin giving form.” The lungs and kidneys mutually promote and transform each other, with metal (lungs) and water (kidneys) nourishing one another, jointly supporting the generation and movement of reproductive essence. Furthermore, the lungs regulate the rhythm and connect with all vessels, moistening the external genitalia to ensure the unobstructed flow of reproductive essence. Therefore, the treatment of oligoasthenospermia should emphasize the simultaneous regulation of the lungs and kidneys. By adjusting the transformative and propelling functions of lung qi, as well as the forming and storing mechanisms of kidney essence, a balance between Yin and Yang can be achieved. This approach offers new perspectives for the clinical diagnosis and treatment of oligoasthenospermia. Hence, oligoasthenospermia can also be treated from the perspective of the lungs.
