Abstract: Non-traumatic Osteonecrosis of the Femoral Head (NONFH) is a refractory orthopedic disease, predominantly affecting young and middle-aged adults with a high disability rate. Western medicine lacks specific conservative treatment options, and surgical treatments have multiple limitations. As an important component of Traditional Chinese Medicine (TCM) pathogenesis theory, the Deficiency-Qi Stagnation theory elaborates the complex, intertwined and mutually causal relationship between deficiency-qi and stagnation. Based on this theory, this study explores the core pathogenesis and therapeutic methods of NONFH, holding that the disease takes insufficiency of healthy qi and deficiency of liver and kidney as the root cause, qi stagnation as well as phlegm and stasis formation as the key factors promoting disease progression, and prolonged disease involving collaterals with intermingling of phlegm and stasis as the underlying cause of its intractability. Correspondingly, a stage-based syndrome differentiation treatment system is established, including replenishing qi to strengthen healthy qi and nourishing liver and kidney, soothing liver to regulate qi and resolving phlegm to remove stasis, and promoting blood circulation to unblock collaterals and nourish bones. This provides a new perspective and approach for the TCM treatment of NONFH.
