Abstract: Pneumonia is a common and prevalent respiratory disease in children, and it remains a leading cause of hospitalization and mortality among children under five years of age worldwide, posing a serious threat to pediatric health. Multiple international studies in recent years have demonstrated that the pathogen spectrum of childhood community-acquired pneumonia (CAP) has gradually shifted from bacterial predominance to viral predominance. Among viral community-acquired pneumonia cases, respiratory syncytial virus (RSV) exhibits the highest detection rate. With advances in pathogen detection technologies, in-depth investigations into pathogenesis, and the launch of novel preventive agents in recent years, the diagnosis, treatment, prevention and control system for pediatric RSV pneumonia has been continuously improved. Based on domestic and international research findings published in recent years, this review summarizes epidemiological characteristics, pathogenic mechanisms, clinical diagnosis, therapeutic strategies, innovative preventive approaches, as well as existing controversies and prospects, aiming to provide evidence-based references for standardized diagnosis, treatment, and early prevention and control of pediatric RSV pneumonia.
