Abstract: Central nervous system neurodegenerative diseases (CNS NDDs) are a group of chronic neurological disorders characterized by progressive loss of neurons. They seriously endanger the health of the elderly population, but their pathogenesis has not yet been fully elucidated. N6-methyladenosine (m6A) methylation, as the most common post-transcriptional modification in eukaryotic mRNA, is widely involved in key processes such as nervous system development, inflammatory response, cell apoptosis, and protein homeostasis. Abnormal m6A modification exists in various CNS NDDs such as Alzheimer''s disease and Parkinson''s disease, which exacerbates neuronal damage, abnormal protein aggregation, and neuroinflammation, promoting disease progression. Currently, Western medicine still lacks a cure. Recent studies have found that some traditional Chinese medicines, with their advantages of multiple components, multiple targets, and overall regulation, have been proven to exert neuroprotective effects by targeting m6A methylation modification. This article systematically reviews the molecular mechanism of m6A methylation modification and its regulatory role in CNS NDDs, and focuses on summarizing the research progress of active ingredients of traditional Chinese medicines and external treatment methods in regulating m6A methylation, with the aim of providing theoretical basis and research ideas for targeting m6A methylation in the prevention and treatment of CNS NDDs by traditional Chinese medicine.
