    Abstract: Obstructive sleep apnea-hypopnea syndrome (OSAHS) is a significant factor driving the development of various cardiometabolic diseases (CMDs). Given the complex associations between OSAHS and CMDs such as hypertension and diabetes, developing biomarkers for comprehensively assessing metabolic health and cardiovascular risk is particularly important. The cardiometabolic index (CMI), which integrates parameters such as triglycerides, high-density lipoprotein cholesterol, and waist-to-height ratio, provides a more sensitive and comprehensive tool for evaluating metabolic risk. This review aims to explore the pathophysiological links between CMI and OSAHS, its efficacy as a predictive marker for OSAHS, and its role in risk stratification for OSAHS-related cardiometabolic complications. It seeks to elucidate the clinical significance of CMI in the management of OSAHS, identify the limitations of current research, and outline future research directions, in order to provide new perspectives and theoretical foundations for the early identification, precise intervention, and improved prognosis of OSAHS.
