    Abstract: Polycystic ovary syndrome (PCOS) is a highly heterogeneous reproductive endocrine and metabolic disorder. The understanding of its pathology has shifted from a focus on abnormalities in the reproductive axis to a new paradigm centered on metabolic dysregulation. The pathogenesis of PCOS is closely linked to key mechanisms including insulin resistance, the vicious cycle of lipid metabolism disorders and chronic inflammation, the regulatory role of the gut microbiota-metabolism axis, and androgen-driven metabolic reprogramming. Based on these mechanisms, this review explores a range of metabolic intervention strategies—from lifestyle modifications and pharmacotherapy to emerging therapies—to provide a theoretical foundation for the clinical management of PCOS.
