Research Progress on the Core Metabolic Pathological Mechanisms and Treatment Strategies of Polycystic Ovary Syndrome

• 摘要：多囊卵巢综合征（polycystic ovary syndrome, PCOS）是一种高度异质性的生殖内分泌代谢性疾病，其病理认知已从生殖轴异常转向为以代谢障碍为核心的新范式。PCOS发生发展与胰岛素抵抗、脂代谢异常与慢性炎症的恶性循环、肠道菌群-代谢轴的调控作用、雄激素驱动的代谢重编程等核心病理机制密切相关。基于上述机制，探讨从生活方式、药物治疗、新兴疗法等一系列的代谢干预策略，为PCOS临床管理提供理论依据。

  Abstract: Polycystic ovary syndrome (PCOS) is a highly heterogeneous reproductive endocrine and metabolic disorder. The understanding of its pathology has shifted from a focus on abnormalities in the reproductive axis to a new paradigm centered on metabolic dysregulation. The pathogenesis of PCOS is closely linked to key mechanisms including insulin resistance, the vicious cycle of lipid metabolism disorders and chronic inflammation, the regulatory role of the gut microbiota-metabolism axis, and androgen-driven metabolic reprogramming. Based on these mechanisms, this review explores a range of metabolic intervention strategies—from lifestyle modifications and pharmacotherapy to emerging therapies—to provide a theoretical foundation for the clinical management of PCOS.
