Abstract: Interstitial lung diseases (ILD) and obstructive sleep apnea (OSA) are common respiratory diseases that often coexist and exhibit complex bidirectional pathological interactions, significantly affecting disease progression and long-term prognosis. This has become a focus in both clinical and research fields. This article systematically reviews the epidemiological characteristics of ILD combined with OSA, with a focus on elucidating the pathophysiological mechanisms of their interactions, analyzing the limitations of current clinical screening and diagnostic approaches as well as novel strategies, and summarizing the current status and controversies regarding first-line and potential treatment methods. Based on existing international research progress, the article identifies current research gaps and provides references for precise screening, individualized treatment, and future research directions for ILD combined with OSA, aiming to optimize clinical diagnosis and treatment pathways and improve patient outcomes

Key words: Interstitial lung disease; obstructive sleep apnea; interaction
