Abstract: Chronic venous edema of the lower limbs is fundamentally rooted in deficiency of the Middle Qi, disruption of its ascending-descending movement, and constraint of Liver Qi failing to maintain free flow. Over time, this leads to fluid accumulation and blood stasis in the collaterals, constituting a deficiency-excess complex pattern. Therefore, guided by the principle that Earth (Spleen) obtains free flow through Wood (Liver), treatment should focus on the Liver and Spleen systems. The therapeutic strategy emphasizes activating the central pivot and relieving wood constraint as its core, integrating methods to tonify qi and invigorate blood, fortify earth (spleen) and strengthen the middle, and soothe the liver to promote water metabolism. This aims to restore the pivotal function of the Middle Jiao and the free coursing of Liver-Wood, thereby ensuring the unimpeded circulation of the unitary qi. When fluids and blood resume their proper pathways, edema resolves naturally.
