Abstract: Objective To investigate the application effect of Laifu decoction on chronic heart failure (CHF) with yuan qi deficiency based on the Liver Governs Collapse. Methods From Feb, 2024 to Oct, 2025, 64 patients with CHF of yuan qi deficiency in our hospital were randomly divided into control group (32 cases) and traditional Chinese medicine group (32 cases). The control group was treated with conventional western medicine, the traditional Chinese medicine group was administered Laifu decoction on top of the control group. The therapeutic effect, TCM syndrome score, cardiac function and glucose and lipid metabolism levels were compared between the two groups. Results The total effective rates of the traditional Chinese medicine group and the control group were 87.50% (21/32) and 65.63% (28/32) respectively, and the traditional Chinese medicine group was higher than that of the control group (P<0.05). After treatment, the TCM syndrome scores of both groups decreased compared with those before treatment, and the decrease was greater in the TCM group (P<0.05). Compared with before treatment, the level of B-type natriuretic peptide in the traditional Chinese medicine group decreased after treatment (P<0.05), There was no difference in the levels of troponin I between the two groups before and after treatment(P>0.05). Compared to their baseline, the control group showed a significant reduction in insulin (INS) and insulin resistance index (HOMA-IR) levels post-treatment (P<0.05). In contrast, the levels of INS, HOMA-IR, glucose, and free fatty acids showed no significant intergroup differences at either time point between the two groups(P>0.05). Conclusion Based on the Liver Governing Dispersing theory, Laifu decoction in the treatment of CHF patients with deficiency of yuan qi could reduce the TCM syndrome score, improve heart function, and had a good clinical effect.
