  Abstract: 糖尿病性骨折愈合迟缓是糖尿病严重的骨折并发症，目前治疗效果有限。基于此，本文提出以“虚、瘀、痰、毒”为核心病机，系统阐述其病理演变规律。认为本病以五脏亏损、肾精亏虚为发病之本，瘀血阻络为初期病理表现，痰浊凝滞为病理进展，而痰瘀化毒为病情恶化的关键。据此确立“补肾安神、活血化瘀、涤痰解毒”的综合治疗原则，结合现代药理研究证实其作用机制，为糖尿病性骨折愈合迟缓的中医诊疗提供理论依据与实践指导。
