Abstract: Abstract: Myopia is the most common type of refractive error worldwide, with its pathogenesis involving multiple processes including scleral extracellular matrix (ECM) remodeling, changes in ciliary muscle function, and regulation of choroidal blood flow. Myosin light chain (MLC), a key regulatory subunit of myosin complexes, dynamically controls cytoskeletal rearrangement, cell contraction, and signal transduction through phosphorylation and dephosphorylation. This review systematically summarizes the molecular structure and regulatory mechanisms of MLC, elucidates the multifaceted roles of MLC-related signaling pathways in scleral fibroblast-to-myofibroblast transdifferentiation, ECM degradation, ciliary body contraction and fibrosis, and choroidal blood flow regulation, and provides an overview of recent advances in intervention strategies such as ROCK inhibitors, MLCK inhibitors, and electroacupuncture. The aim is to offer insights for a deeper understanding of myopia''s underlying mechanisms and to inform the development of improved prevention and treatment approaches.
