Abstract: Objective: To evaluate the clinical efficacy of fumigation and washing with Quyu Tongluo Decoction combined with methylcobalamin tablets in treating blood stasis-type diabetic peripheral neuropathy (DPN). Methods: A retrospective analysis was conducted on 64 patients diagnosed with blood stasis-type DPN and treated at Lishui Hospital of Traditional Chinese Medicine between October 2023 and May 2024. This observational study did not involve randomization; instead, it analyzed existing clinical outcomes to trace prior treatment exposures. Patients were divided into a control group and a treatment group, each comprising 32 cases, based on whether they received fumigation and washing with Quyu Tongluo Decoction. Both groups received standard glucose-lowering therapy. The control group took oral methylcobalamin tablets, while the treatment group additionally underwent external fumigation and washing with Quyu Tongluo Decoction. The treatment duration for both groups was four weeks. Outcomes assessed included TCM syndrome scores, Toronto Clinical Scoring System (TCSS) scores, therapeutic effects according to TCM syndrome, nerve conduction velocity, and serum levels of tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6). Results: The overall effective rate in the treatment group was 93.7%, significantly higher than the 75.0% in the control group (P < 0.05). After treatment, both groups showed significant reductions in TCM syndrome scores, TCSS scores, and inflammatory markers TNF-α and IL-6, as well as improvements in nerve conduction velocity compared to baseline (P < 0.05). Moreover, the improvement in the treatment group was significantly greater than that in the control group (P < 0.05). Conclusion: Adding fumigation and washing with Quyu Tongluo Decoction to conventional glucose-lowering therapy and methylcobalamin treatment can more effectively alleviate clinical symptoms, reduce inflammation, improve nerve conduction velocity, and is safe, making it worthy of clinical promotion.
