Status of Clinical Evaluation and Intervention for Neonatal Coagulation Critical Values

    Abstract: Neonatal coagulation system is characterized by developmental hemostasis, manifested as immature hepatic synthetic function, insufficient vitamin K storage, low levels of coagulation factors, and relatively active fibrinolytic system. Under the influence of pathological factors such as hypoxia, infection, prematurity, shock and acidosis, neonates are prone to coagulation disorders, and some reach laboratory critical values, which are closely associated with adverse outcomes including intracranial hemorrhage, disseminated intravascular coagulation and death. The criteria for coagulation critical values in adults are not applicable to neonates. At present, there is still a lack of unified criteria for gestational age- and postnatal age-specific critical values and intervention strategies for neonates at home and abroad. Based on recent domestic and international literature, this article reviews the physiological and pathological characteristics of the neonatal coagulation system, current status of critical value definition, clinical features and intervention strategies, so as to provide references for clinical practice.
