An Exploration of the Pathogenesis of Asthma Involving

Abstract: Objective: To explore the theoretical basis and clinical application of the method of mediating the middle jiao and unblocking/regulating the sanjiao based on the theory of the middle jiao as the pivot in the treatment of wheezing disorder (bronchial asthma).Methods: The theoretical origins of the middle jiao pivot mechanism and sanjiao qi transformation were systematically reviewed. The pathological mechanism of dysfunction of the middle jiao pivot leading to disordered sanjiao qi transformation in the pathogenesis of wheezing disorder was elucidated. One typical proven case was provided to support clinical efficacy.Results: It is proposed that the key pathogenesis of wheezing disorder lies in the failure of the middle jiao to mediate its pivotal role, leading to latent phlegm-rheum, stagnation of sanjiao qi transformation, and the formation of superficial manifestations such as intermingled phlegm and blood stasis, as well as wind-induced spasmodic constraint. The therapeutic principle should focus on restoring the ascending and descending functions of the middle jiao, while simultaneously facilitating the dispersing and descending of the upper jiao and the qi-receiving function of the lower jiao. The presented case demonstrated that this method can improve the Asthma Control Test score and forced expiratory volume in one second (FEV?), and assist in step-down therapy of inhaled corticosteroids.Conclusion: The method of mediating the middle jiao and unblocking/regulating the sanjiao provides a holistic differentiation and treatment approach for patients with wheezing disorder accompanied by gastrointestinal dysfunction. Its mechanism may be related to regulating the gut-lung axis, though further validation through large-scale clinical studies is required.
