Abstract: SBased on the theory of interdependence of ascending and descending in traditional Chinese medicine, this paper discusses the pathogenesis and treatment approach of functional constipation. Functional constipation is a functional intestinal disorder primarily characterized by difficulty in defecation, reduced stool frequency, or dry and hard stools. Its core pathological location is the spleen, stomach, and large intestine, with the fundamental pathogenesis being the dysfunction of ascending and descending of qi in these organs. When the spleen fails to ascend, clear yang does not rise and transportation is impaired; when the large intestine fails to descend, waste products accumulate and transmission is hindered. These factors, along with liver qi stagnation and kidney yang deficiency, ultimately lead to the accumulation of waste products in the intestines and the development of constipation. Targeting the core mechanism of ascending and descending dysfunction, treatment methods such as ascending the clear and descending the turbid, strengthening the spleen, regulating qi, unblocking the bowels, warming yang, and nourishing yin are employed to restore normal physiological function and improve defecation. This provides a new theoretical reference for the TCM syndrome differentiation and treatment of functional constipation.
