The efficacy and safety impact of the modified bed-mounted exercise bike program on patients with AECOPD and respiratory failure

Abstract: Abstract?Objective? Observation on the therapeutic effect of using non-invasive ventilator combined with modified bed-mounted pedal exercise in patients with AECOPD complicated with respiratory failure.?Method A retrospective study was conducted, collecting the medical records of patients who received treatment at the authors institution, from January 2023 to December 2024. The patients were divided into the observation group and the control group based on different treatment methods. Both groups of patients received routine treatment according to the guidelines, while the control group intermittently used NIV treatment at the same time. The observation group combined with modified bed-mounted pedal exercise therapy. The pulmonary function, blood gas indicators, daily living activity ability, muscle strength of both upper limbs and lower limbs, and adverse reactions of the two groups of patients were compared.??Results After treatment, the PaCO2 levels of the two groups of patients decreased compared to before treatment, while the PEF, FEV1, FEV1/FEV ratio, PaO2 levels, and Barthel index increased compared to before treatment. Moreover, the PaCO2 levels, non-invasive ventilation time, and hospitalization time in the observation group were lower than those in the control group, while the PEF, FEV1, FEV1/FEV ratio, PaO2 levels, and Barthel index were higher than those in the control group (P<0.05). There was no significant difference in the incidence of pipeline detachment, skin injury, and arrhythmia between the observation group and the control group (P>0.05). The tracheal intubation rate in the observation group was lower than that in the control group (P<0.05).?Conclusion The combined modified bed-mounted exercise bike program can improve the lung function and blood gas indicators of patients with AECOPD and respiratory failure, reduce the duration of non-invasive ventilation and hospital stay, and enhance the muscle strength of both upper and lower limb groups.
