Abstract: Acute-on-chronic liver failure (ACLF) is an acute liver injury superimposed on a chronic liver disease, with jaundice and coagulation dysfunction as the main clinical manifestations. Some patients also suffer from multi-organ failure, leading to a high mortality rate. The pathogenesis of ACLF involves the activation of immune cells and inflammatory responses. Immune cells play a key role in ACLF, including monocytes, macrophages, T lymphocytes, dendritic cells, etc. These cells secrete cytokines, participating in the repair and injury of hepatocytes. Traditional Chinese medicine (TCM) has the characteristics of multiple pathways and multiple targets. It can regulate the relevant factors of the immune system to improve the imbalance of inflammatory responses in ACLF, thereby achieving the goal of preventing and treating ACLF. Therefore, this article starts from the relationship between immune cells and ACLF, elaborates on the role of immune cells in ACLF, and how TCM intervention regulates the functions of immune cells and the expression of cytokines, providing a new perspective for the treatment of ACLF.
