Abstract: Following diabetic osteoporosis, hyperthyroidism osteoporosis (HOP) has become another research hotspot and cutting-edge direction in the field of metabolic bone diseases. Modern medicine holds that HOP is closely related to the direct acceleration of bone metabolism by thyroid hormones, resulting in greater bone resorption than bone formation. There is no exact disease name corresponding to this condition within the modern theoretical system of traditional Chinese medicine. However, based on its symptom manifestations, it can be classified under the categories of gall qi and bone atrophy. Based on the theory of liver and kidney having the same origin, this article systematically explores its etiology and pathogenesis. It holds that this disease begins with liver qi stagnation, which then transforms into fire. Excessive fire consumes blood, and over time, it burns kidney Yin and depletes essence and blood, eventually leading to kidney essence deficiency. The kidney governs the bones and generates marrow. When the bones dry up and the marrow decreases, it results in bone atrophy. Conversely, insufficiency of kidney essence also leads to deficiency of liver blood. The liver loses its nourishment, and Yin fails to control Yang, thereby aggravating the hyperactivity of liver Yang and the upward inflammation of liver fire, forming a vicious cycle where hyperthyroidism and osteoporosis interact with each other. The treatment should be based on the fundamental principles of soothing the liver and clearing heat, nourishing water and retaining wood, and replenishing essence and strengthening bones, and be carried out in stages. This article aims to provide a systematic theoretical basis and clinical thinking for the diagnosis and treatment of HOP in traditional Chinese medicine.
