Meta-analysis and Trial Sequential Analysis of Traditional Chinese Medicine External Application for the Treatment of Infusion Phlebitis

• 摘要：目的：系统评价中药外敷对比常规西药治疗输液性静脉炎的临床疗效。方法：检索国内外数据库中相关随机对照试验（RCTs）。采用Cochrane偏倚风险评估工具评价纳入研究的方法学质量，使用RevMan 5.4软件进行Meta分析，并应用试验序贯分析( trial sequential analysis, TSA)和GRADE进行证据质量评级。结果：最终纳入15项RCTs（1435例）。 Meta分析结果显示，中药外敷能显著提高临床总有效率，缩短疼痛缓解时间、疼痛消失时间和红肿消退时间，并降低VAS评分（P＜0.05）。TSA结果表明临床总有效率的结论具有统计学可靠性。GRADE评价显示总体证据等级偏低。结论：中药外敷治疗输液性静脉炎总体优于常规西药外用，但鉴于纳入研究的证据质量偏低，此结论尚需更多高质量、大样本研究证实。

  Abstract: Objective To systematically evaluate the clinical efficacy of external application of Chinese medicine compared to conventional Western medicine for treating infusion phlebitis. Methods We searched domestic and international databases for relevant randomized controlled trials (RCTs). A meta-analysis was performed using RevMan 5.4 software, and the quality of evidence was rated using TSA and GRADE tools. Results A total of 15 RCTs with 1435 patients were included. The meta-analysis showed that external application of Chinese medicine significantly increased the overall clinical effective rate and shortened the time to pain relief, pain disappearance, and resolution of redness and swelling. It also lowered VAS scores (P<0.05). The TSA results confirmed that the conclusion regarding the overall clinical effective rate is statistically reliable. The GRADE assessment, however, indicated that the overall quality of evidence was low. Conclusion The external application of Chinese medicine is generally superior to conventional Western medicine for treating infusion phlebitis, showing some clinical application potential. However, due to the low quality of evidence as assessed by GRADE, more high-quality, large-sample, multicenter studies are needed in the future to further validate these findings.
