Efficacy and safety of Yuye decoction with metformin in type 2 diabetes mellitus: A meta-analysis

• 摘要：目的：旨在系统评价玉液汤联合二甲双胍治疗2型糖尿病（type 2 diabetes mellitus,T2DM）的有效性与安全性，为制定T2DM患者的降糖方案提供参考依据。 方法：通过检索中国知网、万方、维普、中国生物医学文献数据库、PubMed、EMbase、Cochrane Library、Web of Science 8个中英文数据库，收集玉液汤联合二甲双胍治疗T2DM的随机对照试验（randomized controlled trial,RCT）。使用Cochrane评价手册5.3风险偏倚评估工具对文献进行质量评价，采用RevMan 5.4.1统计软件进行数据分析与异质性检验，主要结局指标为糖化血红蛋白（glycated hemoglobin A1c,HbA1c），次要结局指标为胰岛素抵抗指数（homeostatic model assessment for insulin resistance,HOMA-IR）、总胆固醇（total cholesterol,TC）、临床有效率及不良反应率。结果：①经筛选后，最后纳入10项RCTs进行Meta分析。②Meta分析显示，试验组HbA1c（MD=-0.92，95%CI-1.58,-0.26）、HOMA-IR（MD=-0.14，95%CI-0.59,0.32）、TC（MD=-0.83，95%CI-1.95,0.29）及临床有效率(OR=3.73，95%CI2.25,6.18)改善方面均优于对照组，差异有统计学意义（P < 0.05）。在不良反应发生率方面，试验组与对照组无明显差异（P＞ 0.05）。结论：玉液汤联合二甲双胍可改善糖脂代谢、胰岛素抵抗，延缓糖尿病的发展，目前尚未发现严重不良反应。但未来仍需要进行大样本、多中心、双盲、高质量的RCTs以验证有关结论。

  Abstract: Objectives: To access the efficacy and safety of Yuye decoction with metformin in the treatment of type 2 diabetes mellitus (T2DM).Methods: A total of 8 databases including CNKI, SinoMed, Wanfang, VIP, Cochrane Library, PubMed, EMbase and Web of Science were retrieved for randomized controlled trial (RCT) of Yuye decoction in the treatment of T2DM. Cochrane handbook 5.3 was used to evaluate the quality of the included studies. RevMan 5.4.1 were employed to analyze data and test heterogeneity. The primary outcome was glycated hemoglobin A1c(HbA1c). Secondary outcomes include homeostatic model assessment for insulin resistance(HOMA-IR),total cholesterol(TC),clinical effective rate and adverse event rate.Results: A total of 10 RCTs were included in the final meta-analysis. The meta-analysis revealed that the improvements of HbA1c(MD=-0.92，95%CI-1.58,-0.26), HOMA-IR(MD=-0.14，95%CI-0.59,0.32),TC（MD=-0.83，95%CI-1.95,0.29）and clinical effective rate(OR=3.73，95%CI2.25,6.18) in the experimental group were better than those in the control group（P < 0.05）. There was no serious adverse effects being observed in both experimental group and control group. Conclusions: The combination of Yuye decoction and metformin could improve blood glucose and lipid profile as well as reduce insulin resistance, and no serious adverse reactions were observed yet. Further validation in the future is required through large-sample, multi-centered and high quality RCTs.
