摘要:目的 蛋白尿是2型糖尿病(type 2 diabetes mellitus,T2DM)早期肾脏微血管损伤的重要特征,既往研究表明人附睾分泌蛋白4(human epididymis protein 4,HE4)与肾脏损伤关系密切,HE4与蛋白尿的关系的报道较少。本研究旨在探讨HE4对T2DM患者蛋白尿的相关性。方法 收集2020年2月至2023年7月九江市第一人民医院收治的147例T2DM患者作为观察组,根据蛋白尿严重程度分为正常白蛋白尿组、微量白蛋白尿组、大量白蛋白尿组三个组,选择性别、年龄相匹配的健康人群50例作为对照组。比较分析各组HE4和临床指标的变化,采用单因素和多因素线性回归分析HE4与蛋白尿的相关性。结果 相关性网络图显示HE4是连接血液白蛋白、尿微量白蛋白、尿微量白蛋白/尿肌酐比值(urine albumin-to-creatinine ratio,UACR)及肾脏功能生物标志物的一个重要节点;相比于对照组,T2DM患者HE4水平为显著上升,单因素和多因素线性回归分析均显示显示HE4与UACR呈正相关;Logistic回归结果显示校正年龄、性别、肾小球滤过率(estimated glomerular filtration rate,eGFR)、血肌酐(serum creatinine,SCr)、血尿素氮(Blood urea nitrogen,BUN)、血尿酸(uric acid,UA)、白蛋白(albumin,ALB)、乳酸脱氢酶(lactate dehydrogenase,LDH)等混杂因素后,HE4水平升高是蛋白尿发生风险的危险因素(OR=1.110,95%CI=1.005 - 1.226)。结论 HE4与UACR呈正相关,其水平升高增大T2DM患者蛋白尿发生风险。
Abstract: Objective Proteinuria is a critical hallmark of early renal microvascular damage in Type 2 Diabetes Mellitus (T2DM). Previous studies have shown a close association between HE4 and renal injury, but there is limited research on the relationship between HE4 and proteinuria. This study aims to investigate the correlation of HE4 with proteinuria in patients with T2DM. Methods A total of 147 T2DM patients were enrolled in the observation group. Participants were stratified into three distinct categories according to the degree of urinary protein excretion: the macroalbuminuric group, the microalbuminuric group, and the normoalbuminuric group. 50 healthy individuals, matched for gender and age, were selected as the control group. Comparative analysis of HE4 levels across groups and their correlation with clinical parameters was conducted using univariate and multivariate linear regression to assess the association between HE4 and proteinuria. Results The correlation network diagram reveals that HE4 functions as a pivotal node linking serum albumin, urine albumin, UACR, and renal function biomarkers. Compared to the control group, HE4 levels significantly elevate in patients with type 2 diabetes mellitus (T2DM). Both univariate and multivariate linear regression analyses demonstrate a positive correlation between HE4 and UACR. Logistic regression analysis, adjusted for confounding factors including age, sex, eGFR, SCR, BUN, UA, LDH, ALB, confirms that elevated HE4 levels pose a risk factor for proteinuria (OR=1.110, 95%CI=1.005 - 1.226). Conclusion Elevated HE4 levels in patients with T2DM are significantly correlated with proteinuria, indicating a certain predictive value for proteinuria.
