血清CXCL14水平与2型糖尿病患者颈动脉粥样硬化斑块的关系

Association Between Serum CXCL14 Levels and Carotid Atherosclerotic Plaques in Patients With Type 2 Diabetes Mellitus

  • 摘要:目的 探讨血清C-X-C基序趋化因子配体14(CXCL14)水平与2型糖尿病(T2DM)患者颈动脉粥样硬化斑块(CAP)的关系。方法 按照2:1比例选取2022年1月~2025年2月我院收治的T2DM患者262例(T2DM组)和同期体检健康者131例(对照组),根据是否存在CAP将T2DM患者分为CAP组和非CAP组。采用酶联免疫吸附法检测血清CXCL14水平。通过斯皮尔曼/秩二列相关分析CAP患者血清CXCL14水平与临床资料的相关性;多因素Logistic回归和限制性立方样条(RCS)回归分析血清CXCL14水平与T2DM患者CAP的关系;受试者工作特征(ROC)曲线分析血清CXCL14水平对T2DM患者CAP的评估能效。结果 与对照组比较,T2DM组血清CXCL14水平升高(t=-12.432,P<0.001)。262例T2DM患者CAP发生率为42.75%(112/262)。与非CAP组比较,CAP组血清CXCL14水平升高(t=-7.530,P<0.001)。CAP患者血清CXCL14水平与糖尿病病程、空腹血糖、糖化血红蛋白、稳态模型评估-胰岛素抵抗指数、低密度脂蛋白胆固醇、超敏-C反应蛋白、白细胞介素-6呈正相关(P均<0.05)。多因素Logistic回归分析显示,调整一般资料、糖脂代谢指标、炎症指标后,CXCL14高(OR=7.855,95%CI:3.403~18.131)为T2DM患者CAP的独立危险因素(P<0.05)。限制性立方样条回归分析显示,调整一般资料、糖脂代谢指标、炎症指标后,CXCL14水平与T2DM患者CAP风险有关(P<0.05),且呈现正向剂量-反应关系(P<0.05)。ROC曲线显示,血清CXCL14水平为1.32ng/ml时,评估T2DM患者CAP的曲线下面积为0.772(95%CI:0.716~0.821),敏感度和特异度分别为0.589、0.820。结论 血清CXCL14水平升高与T2DM患者CAP风险增加呈正相关,且对T2DM患者CAP具有较高的评估能效。

     

    Abstract: Objective To investigate the association between serum C-X-C motif chemokine ligand 14 (CXCL14) levels and carotid atherosclerotic plaques (CAP) in patients with type 2 diabetes mellitus (T2DM). Methods A total of 262 patients with T2DM (T2DM group) and 131 healthy controls (control group) who underwent physical examination at our hospital between January 2022 and February 2025 were prospectively enrolled in a 2:1 ratio. T2DM patients were further divided into a CAP group and a non-CAP group based on the presence or absence of carotid plaques. Serum CXCL14 levels were measured using enzyme-linked immunosorbent assay. The correlation between serum CXCL14 levels and clinical characteristics in CAP patients was analyzed using Spearman or rank biserial correlation analysis. The association between serum CXCL14 levels and CAP in T2DM patients was assessed using multivariate logistic regression and restricted cubic spline (RCS) regression. Receiver operating characteristic (ROC) curve analysis was performed to evaluate the predictive value of serum CXCL14 levels for CAP in T2DM patients. Results Serum CXCL14 levels were significantly higher in the T2DM group than in the control group (t=-12.432, P<0.001). The incidence of CAP among the 262 T2DM patients was 42.75% (112/262). Serum CXCL14 levels were significantly higher in the CAP group compared to the non-CAP group (t=-7.530, P<0.001). In CAP patients, serum CXCL14 levels were positively correlated with diabetes duration, fasting plasma glucose, glycated hemoglobin, homeostasis model assessment-insulin resistance index, low-density lipoprotein cholesterol, high-sensitivity C-reactive protein, and interleukin-6 ( all P<0.05). Multivariate Logistic regression analysis showed that higher CXCL14 levels (OR=7.855, 95%CI: 3.403 to 18.131) were independently associated with increased CAP risk in T2DM patients after adjusting for demographic, metabolic, and inflammatory indicators (P<0.05). RCS regression revealed a significant and positive dose–response relationship between serum CXCL14 levels and CAP risk in T2DM patients after adjustment (P<0.05). ROC curve analysis showed that when serum CXCL14 was 1.32 ng/mL, the area under the curve (AUC) for predicting CAP in T2DM patients was 0.772 (95%CI: 0.716 to 0.821), with a sensitivity of 0.589 and specificity of 0.820. Conclusion Elevated serum CXCL14 levels are positively associated with an increased risk of carotid atherosclerotic plaques in patients with type 2 diabetes mellitus and demonstrate good predictive efficiency.
