Efficacy Analysis of Two Different Endovascular Thrombectomy Approaches for Treating Early-Stage Lower Extremity Deep Vein Thrombosis: A Single-Center Retrospective Study

    Abstract: Objective: To compare and analyze the clinical efficacy and safety differences between AngioJet mechanical thrombectomy and large bore catheter aspiration systems in treating early deep vein thrombosis in the lower limbs. Methods: A retrospective analysis was conducted on 88 patients with deep vein thrombosis in the lower limbs who received mechanical thrombectomy treatment at Li Hui Li Hospital, Ningbo Medical Center from January 2020 to October 2024. The patients were divided into two groups based on surgical method: Group AngioJet (A group) and Group B (negative pressure large bore catheter group). The perioperative indicators (surgical duration, hospital stay), thrombus clearance outcomes (immediate clearance rate, final clearance rate), and postoperative complications were compared between the two groups. Results: The baseline data of both groups were comparable (P>0.05). The success rate of surgery was 100%, and there were no statistically significant differences in hospital stay, surgical duration, postoperative clearance rate, or final clearance rate between the two groups. In terms of complications, the incidence of hematuria in Group A was significantly higher than in Group B, with 3 cases of puncture site bleeding in Group B. Conclusion: Both mechanical thrombectomy methods demonstrated good therapeutic effects in early LEDVT. The AngioJet mechanical thrombectomy system requires vigilance against the risk of hemolysis-related kidney injury, and it is recommended to enhance perioperative hydration and renal function monitoring. Large-bore negative pressure catheters have shortcomings such as puncture site bleeding, cumbersome operation, and venous valve damage. Percutaneous mechanical thrombectomy devices still require continuous improvement and innovation.
