Abstract: Purpose/Significance:To understand the characteristics of chronic health management information, and provide reference for improving the health management information model. Method/Process: The relevant literatures from the self-built database at home and abroad to June 13, 2024 were systematically searched as data sources. Content analysis of the included literature texts was performed on the DiVoMiner? platform. Result/Conclusion: A total of 114 articles were included. According to the over-coded data, the proportion of health risk prediction in the service process of the health management system is the lowest 9.0%, and the service carrier is mainly mobile phone software (52%), of which WeChat accounts for 68%; The collection of health information focused on physiological aspects (68.0%), and the collection method was mainly online (61.5%); Health information was transmitted between hospitals and families (54.9%), usually by telephone (40.7%). Chronic disease health management information shows the characteristics of comprehensive service process, but health risk prediction, psychological and social information collection, and health information interconnection still need to be strengthened.
