Abstract: Purpose/Significance To evaluate the applicability, feasibility and practicability of O2O model in chronic disease management, with a view to providing a basis for the innovation and development of chronic disease management model in China. Method/Process Retrieve domestic and foreign databases, summarize the implementation methods of O2O model and its application status in the management of common chronic diseases, and analyze the existing problems and development countermeasures. Result/Conclusion The O2O model has been applied in the fields of disease monitoring, psychological nursing, health education and follow-up management, which can improve clinical symptoms and improve patients self-management ability. However, the O2O model is still in the initial stage of development, and it is urgent to improve the patient s digital health literacy level, build a unified online platform and improve the information security system.
