Abstract: Objective To understand the influence and mechanism ofworking after retirement labor participation behavior of retired elderly groups on their utilization of medical services, and to provide suggestions for the introduction of progressive delayed retirement policy. Methods Using 2020 CFPS data, Stata17.0 statistical software was used to study the influence of labor participation on medical service utilization in retired elderly people by Logit regression model and propensity score matching (PSM). The three-step regression method and KHB decomposition method were used to explore the mechanism of labor participation affecting the utilization of medical services for the elderly working after retirement. Results The Logit model showed that post-retirement labor participation had a negative impact on the utilization of medical services, and after correcting the endogenous problem, the average treatment effects of post-retirement labor participation on outpatient service utilization and inpatient service utilization were -0.059 and -0.074, respectively, and the effect on male elderly and high-generation support elderly in medical service utilization was more obvious. The mediating effect of self-rated health as an intermediary on the utilization of outpatient services and inpatient services accounted for 77.04% and 40.76% of the total effects, respectively. Conclusion working after retirement can alleviate the pressure of medical resources to a certain extent, and the elderly should be encouraged to participate in labor after retirement, and flexible adjustment of retirement policies should be made according to different groups of people.
