Abstract: Medical insurance is an important part of Chinas social insurance system. Whether it can effectively improve the health level of the insured and the utilization of medical services has been the focus of debate among scholars at home and abroad. Based on the three waves balanced panel data of the Chinese Longitudinal Healthy Longevity Survey(CLHLS) from 2005 to 2018, the time-fixed effect model and system GMM method is used to analyze the effects of the medical insurance for urban and rural residents on the health level and medical service utilization of the insured elderly. The study shows that participating in medical insurance for urban and rural residents could not effectively improve the health level of the elderly, but could improve their utilization of medical services, and the effect is greater for the elderly whose health has deteriorated. Based on this, the paper argues that there is still a certain gap between the actual effect of the medical insurance system for urban and rural residents and its expected goals. On the basis of the great progress made in expanding the coverage of medical insurance, the construction of the medical insurance system for urban and rural residents in the future should shift the focus to improving the level of security and realizing sustainable development. , improving the compensation of medical insurance, and attaching importance to the disease-prevention role.
