Abstract: Objective: To explore the research progress of traditional Chinese medicine for Irritable Bowel Syndrome with predominant Diarrhea (IBS-D), and offer new insights into traditional Chinese medicine (TCM) management of IBS-D. Methods: Relevant literatures concerning TCM in the treatment of IBS-D published from January 2015 to May 2026 were retrieved from CNKI, Wanfang Data, CQVIP and China Biology Medicine disc (CBMdisc). CiteSpace 6.4 R1 software was adopted to conduct visual analysis on publication volume, authors, institutions, keywords and other indicators. Results: A total of 2294 publications were included. The number of publications on traditional TCM for the treatment of IBS-D shows an overall upward trend. Tang Xudong is the most productive author, and Guangxi University of Chinese Medicine is the institution with the largest number of publications. Several closely collaborating research teams have been formed, most of which are partnerships between universities of Chinese medicine and their affiliated hospitals. In recent years, the main research hotspots include acupuncture, Tongxie Yaofang, and intestinal microbiota. Conclusion: Intestinal flora, the brain-gut-microbiota axis, low-grade intestinal inflammation, and the therapeutic mechanisms of TCM against IBS-D represent the future research directions. It is necessary to strengthen academic cooperation and interdisciplinary integration, so as to further promote multi-dimensional and in-depth researches on TCM therapy for IBS-D.
