The Clinical Significance of the Principle “When Jueyin Fails to Respond to Treatment, Seek Therapy from Yangming”

  Abstract: “Jue” denotes “extreme” or “ultimate”, referring to the juncture where two yin energies reach their culmination and yang begins to regenerate. Jueyin governs closure, storage, and discharge regulation, internally harboring ministerial fire. Though positioned at the terminal stage of the six meridians, it serves as the pivotal hub governing the ascending-descending and entering-exiting of qi throughout the body. Jueyin disorders typically manifest as intermingled cold and heat, combined deficiency and excess, and disordered qi movement—often chronic and refractory, constituting genuinely complex clinical challenges. While conventional therapeutic approaches focusing solely on liver regulation (warming, clearing, or calming the liver) frequently yield unsatisfactory results, shifting the therapeutic focus to Yangming regulation often produces immediate and remarkable efficacy. Following this reasoning, this article systematically elucidates the clinical significance of “when Jueyin proves refractory, seek Yangming” by examining theoretical origins, argumentative foundations, clinical differentiation and treatment, and illustrative case studies, aiming to fill existing gaps in Jueyin disorder therapeutics.
