Lipid Profile and Nephropathy Progression in Diabetic Kidney Disease

Abstract: Diabetic kidney disease (DKD) is the most common microvascular complication of diabetes, with progression closely linked to dyslipidemia. Traditional lipid markers have been consistently associated with DKD, yet their predictive performance is limited and do not fully account for residual risk. Emerging lipid biomarkers, including remnant cholesterol, lipoprotein(a) (Lp(a)), and the apolipoprotein B (ApoB)–containing lipoprotein family, have gained focused attention for their potential to more accurately gauge lipotoxic burden, glomerular inflammation, and fibrotic progression. These markers may enhance early warning and risk stratification, guiding timely intervention. This narrative review summarizes current evidence on lipid spectrum components in DKD development, with emphasis on the associations of remnant cholesterol, Lp(a), and ApoB with DKD, and discusses their roles in histopathologic changes, renal function decline, early screening, risk prediction, and dynamic assessment of therapeutic effects.
