Association Between Lipoprotein-to-Uric Acid Ratios and Nonalcoholic Fatty Liver Disease in Newly Diagnosed Type 2 Diabetes Patients

Abstract: Objective To investigate the association and predictive value of various lipoprotein-to-uric acid ratios with nonalcoholic fatty liver disease (NAFLD) in newly diagnosed type 2 diabetes mellitus (T2DM) patients. Methods In this retrospective study, 138 newly diagnosed T2DM patients were enrolled and categorized based on abdominal ultrasound findings into NAFLD (n=106) and non-NAFLD (n=32) groups. Demographic and biochemical parameters were collected to calculate five ratios: TC/HDL-C, TG/HDL-C, LDL-C/HDL-C, UA/HDL-C and TG/ApoA1. Multivariable logistic regression was employed to identify independent associations, with predictive performance evaluated and compared using receiver operating characteristic (ROC) curve analysis and DeLong test. Results The detection rate of NAFLD was 76.81% in this cohort. After adjusting for confounders, all ratios were independently associated with NAFLD (P < 0.05). ROC curve analysis revealed that all five lipoprotein ratios exhibited good predictive value for NAFLD, with TG/ApoA1 demonstrating the largest area under the curve of 0.846 (95% CI: 0.767-0.925). Conclusion In newly diagnosed T2DM patients, the ratios of TC/HDL-C, TG/HDL-C, LDL-C/HDL-C, UA/HDL-C, and TG/ApoA1 are all independently and positively associated with NAFLD and demonstrate good predictive capability. The TG/ApoA1 ratio shows outstanding predictive potential, suggesting its value as a clinical screening biomarker for NAFLD.
