Abstract: Objective: To investigate the association between the uric acid to high-density lipoprotein cholesterol ratio (UHR) and the prevalence of intracranial aneurysm. Methods: A total of 104 patients with intracranial aneurysm and 97 hospitalized patients without aneurysm during the same period were included as controls. Demographic characteristics, lifestyle factors, and laboratory biochemical parameters were collected. Multivariable logistic regression was performed to assess the association between UHR and intracranial aneurysm, with UHR entered into the models both as a continuous variable and as quartiles (Q1-Q4). Restricted cubic spline (RCS) analysis was used to evaluate the dose-response relationship, and receiver operating characteristic (ROC) curves were used to assess model discrimination. Results: In the fully adjusted model, UHR was positively associated with the prevalence of intracranial aneurysm (P<0.05). When UHR was analyzed by quartiles, compared with the Q1 group, the risk of intracranial aneurysm was increased in the Q2 group (P<0.05), while no statistically significant difference was observed in the Q3 group, and the Q4 group showed a borderline association. ROC analysis showed that the area under the curve (AUC) increased from 0.541 to 0.712. RCS analysis did not indicate a significant nonlinear association. Conclusion: Higher UHR was associated with an increased prevalence of intracranial aneurysm and may serve as an auxiliary indicator for risk assessment.
