Analysis of the difference between VCIND and cognitively normal cerebrovascular disease

Abstract: Objective To analyze the differences of cerebral vascular morphology and serologic indexes between non-demented vascular cognitive impairment (VCIND) and cognitively normal cerebral vascular disease. Methods A total of 100 VCIND patients admitted to the Department of Neurology of our hospital from June 2023 to September 2024 were selected as the study group, and 100 cognitively normal cerebrovascular disease patients of the same period were selected as the control group. By means of cerebrovascular angiography and serological examination, the differences of cerebral artery dilation, vascular stenosis, collateral circulation establishment, inflammatory factors and lipid metabolism indexes between the two groups were analyzed. And to explore the relationship between the severity of cognitive impairment and the patients with different responsibility vascular diseases. Results The total proportion of cerebral artery elongation and dilation in the study group was higher than that in the control group (P<0.05). The stenosis free rate of the study group was lower than that of the control group (P<0.05). The establishment rate of collateral circulation in the study group was lower than that in the control group (P<0.05). The levels of inflammatory factors in the study group were lower than those in the control group before admission and treatment (P<0.05). The levels of total cholesterol (TC), low-density lipoprotein (LDL), apolipoprotein B (ApoB) and lipoprotein aLp (a) in the study group were higher than those in the control group before admission and treatment, and the levels of high density lipoprotein (HDL) were lower than those in the control group (P<0.05). Cerebral anterior artery vascular disease had the lowest cognitive function score among all cerebral artery vascular diseases, and there was no statistical significance between groups (P>0.05). Conclusion Through comparative analysis, it was confirmed that patients with cerebral artery elongation and dilation, severe stenosis, no establishment of collateral circulation, obvious inflammatory response and metabolic abnormalities were high-risk groups for the occurrence of VCIND, and early cognitive intervention was needed for these patients to prevent the occurrence of VCIND.
