Clinical value of systemic immune-inflammation index combined with IL-4 and IL-8 in predicting acute lung injury after traumatic brain injury

    Abstract: Objective To evaluate the value of the systemic immune-inflammation index (SII) combined with interleukin-4 (IL-4) and interleukin-8 (IL-8) for predicting acute lung injury (ALI) after traumatic brain injury (TBI). Methods A total of 498 patients with TBI admitted between April 2025 and April 2026 were retrospectively enrolled and classified into an ALI group (n=126) and a control group (n=372) according to whether ALI developed within 7 days after injury. Clinical data were collected, serum IL-4 and IL-8 levels were measured, and SII was calculated. Associations between these indicators and ALI were analyzed. Results The proportions of patients with midline shift and early mechanical ventilation were higher, whereas the oxygenation index was lower, in the ALI group than in the control group (P<0.05). SII, IL-4, and IL-8 levels were higher in the ALI group (P<0.05). After adjustment for midline shift and early mechanical ventilation,and emergency surgery, SII, IL-4, and IL-8 remained associated with ALI after TBI (P<0.05). Receiver operating characteristic curve analysis showed that the areas under the curve for SII, IL-4, and IL-8 were 0.780, 0.796, and 0.790, respectively, and that of the combined model was 0.907. Conclusion Combined assessment of SII, IL-4, and IL-8 has good predictive value for ALI after TBI and may facilitate early clinical risk identification.
