Abstract: Taking the theory of ‘fluid imbalance’ as a starting point, this study explores its correlation with endoplasmic reticulum (ER) stress mechanisms in the onset and progression of dry eye. It posits that disruption of ER homeostasis is a key prerequisite for fluid imbalance, with impaired protein folding and secretion serving as its specific manifestations; inflammatory responses and oxidative stress constitute the key pathological processes; and insufficient lubrication of the eye represents the ultimate clinical manifestation. Elucidating the implications of fluid imbalance at the level of microscopic molecular mechanisms provides a theoretical basis for understanding the pathogenesis of dry eye from the perspective of ‘fluid imbalance’ and for identifying targets for Traditional Chinese Medicine intervention.
