Abstract: Multizonal anogenital intraepithelial neoplasia(MZIN) is a clinically significant yet often overlooked special type of HPV-related precancerous lesion affecting the lower genital tract and anus. Its development is associated with multiple factors, including persistent infection with high-risk HPV, abnormal host immune status, a history of high-grade lesions, and imbalance of the local microenvironment. Because current screening and diagnostic strategies are largely focused on single-site lesions, MZIN still presents major clinical challenges, such as a high rate of missed diagnosis, lack of standardized treatment pathways, and a high risk of recurrence. Comprehensive regional assessment for high-risk populations, strengthened multidisciplinary collaboration, individualized treatment strategies based on lesion grade and involved sites, and standardized long-term follow-up are key to improving the diagnosis and management of MZIN.
