Abstract: Objective: To summarize Professor Yuan Fangs clinical experience and academic thought in treating chronic renal failure (CRF) with Chinese herbal medicine combined with colonic hydrotherapy. Methods: Through clinical apprenticeship, case record analysis, and typical case studies, we systematically reviewed Professor Yuans understanding of the pathogenesis, treatment principles, and formulas for CRF. Results: Professor Yuan believes that the core pathogenesis of CRF is deficiency of the spleen and kidney, with internal accumulation of dampness, turbidity, stasis, and toxin. Accordingly, he established the treatment principle of strengthening the healthy qi and reinforcing the root, as well as clearing the fu-organs and draining turbidity. For internal treatment, the empirical formula Fuzheng Huayu Xiezhuo Decoction (composed of Astragali Radix, Pseudostellariae Radix, Cuscutae Semen, Rhei Radix et Rhizoma, Salviae Miltiorrhizae Radix et Rhizoma, etc.) is used to fortify the spleen and tonify the kidney, activate blood circulation and resolve stasis, and clear the fu-organs to drain turbidity. For external treatment, colonic hydrotherapy is employed to cleanse the intestines and guide pathogenic factors outward. These two methods work synergistically to establish an internal and external simultaneous treatment system. Clinical observation demonstrated that this comprehensive regimen can effectively reduce serum creatinine and urea nitrogen levels, improve clinical symptoms, and delay disease progression. Conclusion: The combination of oral Chinese herbal medicine and colonic hydrotherapy is effective for CRF, fully embodying the therapeutic advantages of Chinese medicine in simultaneous reinforcement and elimination and internal and external simultaneous treatment. It is worthy of clinical reference and promotion.
