Self-management in patients with osteoporotic vertebral compression fractures: a Rodgers evolutionary concept analysis

• 摘要：目的：分析并阐述骨质疏松性椎体压缩性骨折患者自我管理的概念内涵。方法：系统检索中英文数据库中与 OVCF患者自我管理相关的文献，应用Rodgers演化概念分析法对纳入文献进行分析。结果：OVCF患者自我管理的概念属性包括知识驱动性、安全首要性、行为策略性、互动协作性与动态调适性；前因包括疾病相关、个体特征、社会支持以及心理认知因素；结果和意义涉及患者自身、家庭照护者及社会经济三方面。结论：本研究明确了OVCF患者自我管理的概念内涵及边界，构建了包含先决条件、定义性特征及健康结局的概念框架，揭示了其属性及各要素间的序贯与交互关系，为OVCF患者自我管理评估工具开发、护理干预方案构建及相关研究提供了理论依据。

  Abstract: Objective：To analyze and clarify the conceptual connotation of self-management in patients with osteoporotic vertebral compression fracture. Methods：Chinese and English databases were systematically searched to identify studies related to self-management in patients with OVCF. Rodgers'' evolutionary concept analysis method was used to analyze the included literature. Results：The defining attributes of self-management in patients with OVCF included knowledge-driven, safety-oriented, behavior-strategic, interactive and collaborative, and dynamic and adaptive. The antecedents comprised disease-related factors, individual characteristics, social support, and psychological-cognitive factors. The consequences were reflected at three levels: patients, family caregivers, and society. Conclusion：This study clarified the concept and boundaries of self-management in patients with OVCF and constructed a conceptual framework consisting of antecedents, defining attributes, and health outcomes. The framework revealed the multidimensional characteristics of self-management as well as the sequential and interactive relationships among its components, providing a theoretical basis for the development of disease-specific assessment tools, nursing intervention programs, and future research on self-management in patients with OVCF.
