Clinical Efficacy Analysis of Needle Knife Therapy Guided by Musculoskeletal Ultrasound for Residual Low Back Pain after Osteoporotic Vertebral Compression Fracture Surgery

• 摘要：目的：研究肌骨超声引导下针刀治疗骨质疏松椎体压缩性骨折（OVCF）术后残留腰背痛，评估其临床疗效和安全性，期以改进传统针刀，提高临床疗效及安全性，以便其推广。方法：在洪都中医院门诊或住院病人招募例60例OVCF术后残留腰背痛患者，随机分为肌骨超声针刀组及传统针刀组各30例，观察其治疗前后腰背VAS评分及ODI评分，竖脊肌及腰大肌肌肉弹性剪切波速度（SWV），不良反应发生率以及总体有效率，探讨肌骨超声引导下针刀治疗OVCF（骨质疏松椎体压缩性骨折）术后残留腰背痛患临床疗效。结果：治疗后两组腰背VAS评分及SWV值均低于术前,ODI值高于术前（P<0.05）,且肌骨超声针刀组腰背痛VAS与SWV值低于传统针刀组，ODI评分高于传统针刀组，（P<0.05）,肌骨超声针刀组整体有效率高于传统针刀组，不良反应发生率低于传统针刀组，（P<0.05）。结论：肌骨超声引导下针刀能够明显改善OVCF残留腰背痛患者腰背痛，提高生活质量，减轻肌肉紧张度，不良反应较传统针刀更低，安全性高，值得推广。

  Abstract: Objective: To study the effect of musculoskeletal ultrasound-guided needle-knife therapy on residual back pain after osteoporotic vertebral compression fractures (OVCF), and to evaluate its clinical efficacy and safety, with the aim of improving the traditional needle-knife technique and enhancing its clinical efficacy and safety, so as to promote its application. Methods: 60 patients with residual back pain after OVCF surgery were recruited from the outpatient or inpatient department of Hongdu Central Hospital. They were randomly divided into the musculoskeletal ultrasound needle-knife group and the traditional needle-knife group, with 30 cases in each group. The VAS scores and ODI scores of the back and waist before and after treatment, the shear wave velocity (SWV) of the erector spinae and the quadratus lumborum muscles, the incidence of adverse reactions, and the overall effective rate were observed. The clinical efficacy of musculoskeletal ultrasound-guided needle-knife therapy for residual back pain after OVCF (osteoporotic vertebral compression fractures) was explored. Results: After treatment, the VAS scores and SWV values of the back and waist in both groups were lower than those before the operation, and the ODI values were higher than those before the operation (P < 0.05). The VAS and SWV values of back and waist pain in the musculoskeletal ultrasound needle-knife group were lower than those in the traditional needle-knife group, and the ODI score was higher than that in the traditional needle-knife group (P < 0.05). The overall effective rate of the musculoskeletal ultrasound needle-knife group was higher than that of the traditional needle-knife group, and the incidence of adverse reactions was lower than that of the traditional needle-knife group (P < 0.05). Conclusion: Musculoskeletal ultrasound-guided needle-knife can significantly improve the back pain of patients with OVCF residual back pain, improve quality of life, reduce muscle tension, and has lower adverse reactions and higher safety compared with traditional needle-knife, which is worthy of promotion
