A Clinical Application Study of TCM-Based Fatigue Management Intervention Using the Transtheoretical Model in Patients Undergoing Hematopoietic Stem Cell Transplantation

    Abstract: AbstractObjective To develop a Traditional Chinese Medicine (TCM)–based fatigue management program for patients undergoing hematopoietic stem cell transplantation (HSCT) guided by the transtheoretical model of change (TTM), and to evaluate its practical effectiveness in improving patient outcomes. Methods The Delphi method was employed to design a structured fatigue management program based on the TTM. Two rounds of expert consultation were conducted to ensure scientific rigor and clinical applicability. From January to December 2024, a total of seventy HSCT inpatients admitted to the First Affiliated Hospital of Zhejiang Chinese Medical University / Zhejiang Provincial Hospital of Chinese Medicine of TCM were recruited. Participants were randomly assigned, using a random number table, to either a control group (n=35) or an intervention group (n=35). The control group received routine ward nursing, whereas the intervention group received the TCM fatigue management program based on the TTM in addition to standard care. The levels of fatigue, general self-efficacy, and quality of life were assessed before the intervention and again after 90 days of implementation using validated assessment tools. Results During the study, one participant in the intervention group withdrew due to disease progression. After 90 days of intervention, the fatigue score in the intervention group was significantly lower than that of the control group (P<0.05), while both general self-efficacy and quality of life scores were significantly higher than those observed in the control group (P<0.05). These results indicate that the proposed program effectively reduced fatigue symptoms and promoted positive psychological adaptation among HSCT patients. Conclusion The TCM fatigue management program for HSCT patients developed on the basis of the TTM demonstrated both scientific validity and safety. Its implementation effectively alleviated fatigue, enhanced patients’ self-efficacy, and improved overall quality of life.
