Abstract: Objective: To explore the expression and clinicopathological significance of secretory leukocyte protease inhibitor (SLPI) in cervical adenocarcinoma (AC) and adenocarcinoma in situ (AIS) of the cervix, and to analyze the correlation between SLPI expression and human papillomavirus (HPV) infection. Methods:Immunohistochemical staining of SLPI protein and HPV gene detection were performed on 87 cases of cervical glandular lesion tissues diagnosed in the Department of Pathology, Changsha Central Hospital from 2019 to 2022. The expression of SLPI in AC, AIS, and normal cervical glands was explored, and the correlation between its expression level and the clinicopathological characteristics of AC patients and HPV infection was analyzed. Results:The expression of SLPI in AIS and AC tissues was lower than that in normal cervical tissues (P < 0.05). There was a negative correlation between the expression of SLPI and HPV infection (r=-0.353, P < 0.05). The expression of SLPI was significantly correlated with histological differentiation, HPV infection, vascular invasion, lymph node metastasis, Silva classification, and FIGO stage of AC (P < 0.05).Conclusion: SLPI is low - expressed in cervical adenocarcinoma, which is closely related to the invasive biological and pathological characteristics. The low expression of SLPI is related to HPV infection, suggesting that SLPI may participate in the progression of cervical adenocarcinoma as a tumor - suppressor factor. The decrease in SLPI expression may indicate HPV infection. SLPI is expected to become a potential biomarker for the diagnosis, treatment, and prognosis of cervical adenocarcinoma.
